We want to make sure your first visit to the office runs as smoothly as possible. For your convenience, you can download our new patient forms ahead of time and bring them with you to your first appointment.

If this is your first visit with Dr. Sharpe MD, you will need to bring a few other important items with you:

• Insurance card
• List of current medications and supplements
• Copies of records from previous treatments and facilities

Insurance

During your initial visit, Bella Dermatology will gather all of your insurance details. Our office accepts most major insurances. Please be sure to call our office ahead of time in order to verify that we accept your current insurance provider. We also gladly accept cash and credit card payments at the time of service. Any costs not covered by your insurance policy will become your responsibility and we will consult with you on resolving those.
